A Pap smear detects changes in the cells of the cervix before they become a problem. This guide covers how often it is due, how to prepare and what each result means.
How often is it due?
According to the USPSTF:
- Ages 21 to 29: cytology every 3 years
- Ages 30 to 65: cytology every 3 years, or every 5 when combined with HPV testing
- Before 21: screening is not recommended
- After 65: it can be stopped if previous results were normal
An abnormal result shortens the interval. Your history sets the timing, not the calendar.
How to prepare
- Do not use douches, vaginal suppositories or creams in the 48 hours before
- Avoid sex for 48 hours beforehand
- If possible, do not come during your period
- Bring the date of your last period
- If you had an abnormal result before, bring the report
Skipping these can make the sample insufficient and force a repeat.
Does it hurt?
It should not. A speculum is placed and a sample of cells is taken with a small brush. The sensation is pressure, not pain, and it lasts a few seconds. If it is very uncomfortable, say so: speculum size and positioning can be adjusted.
Light spotting afterwards is normal and lasts a day or two.
What each result means
| Result | What it means |
|---|---|
| Normal | No abnormal cells were seen |
| ASC-US | Atypical cells of unclear significance; usually paired with HPV testing |
| LSIL | Mild changes, often from HPV, which frequently resolve on their own |
| HSIL | Higher-grade changes; referred for colposcopy |
| Insufficient | Could not be read, usually from blood or too little sample; must be repeated |
An abnormal result does not mean cancer. It means follow-up is due, and most mild changes go away without treatment.
About HPV
Most cervical changes are caused by human papillomavirus. It is a very common infection and in most cases the body clears it on its own. HPV testing is combined with cytology from age 30 because together they detect what matters better.
