What to bring
- The district or league form, if it has already been given to you
- Vaccination card
- Medication list, inhalers included

- If there is asthma, allergies or any heart condition, the doctor's reports
Without the right form you have to come back: every district and league has its own. In Texas, school sports use the UIL forms.
What is a sports physical looking for?
Above all, heart conditions that could cause trouble with exertion. That is why the questions cover fainting during exercise, chest pain when running, shortness of breath out of proportion to the effort, or a family history of sudden death. If any of that comes up, a cardiology referral happens before signing.
Previous injuries that never healed properly are also reviewed, since they are the most common reason injuries repeat.
When should it be done?
Before the season starts, not the week before the first game. If something turns up that needs further assessment, there is room to resolve it without missing the start of the season.
Walk-in and no insurance
Monday through Sunday, 9 AM to 9 PM. If vaccines are also missing, they are given in the same visit under vaccines.
