The immigration medical exam (Form I-693) is part of the adjustment of status process. At Clínica Hispana Nueva Salud Little York it is done walk-in, in Spanish, and the form is handed over inside the sealed envelope USCIS requires.
What does the I-693 exam include?
- Review of medical and vaccination history
- Complete physical exam
- Tuberculosis testing: for immigration the IGRA blood test is used
- Blood testing for syphilis and gonorrhea depending on age
- Administration of any missing vaccines
- Form I-693 completed and sealed
What should I bring?
- Photo ID and passport
- Your vaccination record, if you have it, even from your home country
- A list of the medication you take
- Medical reports for any chronic condition
- If you have had a tuberculosis test before, the result
Bringing the vaccination card is what saves the most time and money: without it, doses you may already have had must be repeated.
About the sealed envelope
The form is handed over in a closed, signed envelope. Do not open it. If the envelope reaches USCIS opened, the exam is considered invalid and has to be repeated. Keep it exactly as given until you file it.
Rules worth checking before you go
USCIS rules change, so it is worth checking them on the official I-693 form page before your visit. Two points have changed in recent years: the COVID-19 vaccine is no longer a requirement, and USCIS asks for the I-693 to be filed together with Form I-485 rather than separately.

How long does it take?
The exam is done in one visit. Vaccines requiring several doses and lab work may need a second appointment, and the envelope is handed over once the whole file is complete. You are told the date at the first visit.

Neither. Walk-ins are seen Monday through Sunday, 9 AM to 9 PM. Ask for the cost of the exam and of the vaccines separately before starting.
