Not every check belongs to every age. This guide sorts by life stage what is worth checking, how often, and which signs move the visit forward, so nothing is done in excess and nothing due is skipped.

Ages 20 to 29

The priority is cervical cancer and sexual health. The USPSTF recommends cytology every 3 years starting at 21. Screening is not recommended before that age, although a visit is warranted if there are symptoms.

• Pap smear every 3 years from age 21
• Sexually transmitted infection testing based on sexual activity
• Contraception: choosing a method, starting it and reviewing it
• Review of irregular cycles, period pain or hormonal acne

Ages 30 to 39

Cervical screening continues, with the option to space it out, and metabolic factors start to matter.

• Cytology every 3 years, or every 5 when combined with HPV testing
• Blood pressure, glucose and cholesterol in general blood work
• Thyroid function if there is fatigue, hair loss or weight change (thyroid)
• Preconception review if pregnancy is planned

Ages 40 to 49

Breast screening begins. The USPSTF recommends a mammogram every two years from age 40 through 74.

• Biennial mammogram from age 40
• Cervical screening continues to age 65
• Review of heavy or irregular bleeding, common at this stage
• Pelvic ultrasound for pain, bleeding or suspected fibroids

Age 50 and beyond

Menopause changes both the symptoms and the risk map.

• Hot flashes, vaginal dryness, insomnia: these have treatment, they do not have to be endured
• Any bleeding after menopause is always looked at, without exception
• Bone density, depending on risk factors
• Blood pressure, blood sugar and cholesterol more often

Signs that move the visit forward at any age

• Bleeding between periods, after sex or after menopause
• Pelvic pain that does not ease
• Discharge with a bad smell or intense itching
• A breast lump, dimpled skin or nipple discharge
• Burning when urinating with fever or lower back pain
